This professional dog walking registration form is ideal to use to enrol a new dog walking client. It is an editable dog walking registration form that you can update in Word as required. In addition, you can print off the PDF version.

• Created using a table in Microsoft Word – you should be proficient in Word to customize this form.
• The checkboxes may not be clickable if using an older version of Word.
• PDF to be used as a printable but you can use software such as PDFEscape to make it fillable.

This is a form ideally best used for printing out rather than being filled out electronically.
It is using Word tables which may be difficult for an inexperienced Word user to change but changing the text should be straightforward. If using Google Docs the paws watermark will be too dark as it is not supported.
The checkboxes may not be clickable if using an older version of Word.
